
The NC Coalition is conducting a survey to collect 5,000 stories. These surveys will be compiled, summarized and analyzed to provide policymakers and funders with specific, current data on the impact of the choices made to cut services to vulnerable individuals.
Please take a few minutes to complete the survey to help us as we work to continue services.
There are three different surveys. Please select the survey that best reflects your role.
For Individuals who have lost some or all of their services within the MHDDSA system (includes residential and vocational). If you are concerned that you will lose services but haven't yet, complete the survey now and then again if or when you lose some or all of your services.
For Providers of the MHDDSA system (employers) who have cut services, employee rates, benefits, hours or related services.
For Employees (current or former employees of MHDDSA providers) who have lost jobs or experienced reductions in hours, salary or benefits.
The survey will be active between December 1, 2009 and June 30, 2010. As of December 12, 400 stories had been submitted.
